Our Vital Records department is the office you would contact to request a certified copy of a death certificate. This service is available in each of our offices.
Once you know that you need a certified copy of a death certificate, the next step is to determine where the event occured. Death records are filed according to the state where the event occured. If the event occured in another state, then a certified copy of the record can only be obtained by applying directly to that state.
How to Apply:
Fill out an application at your local health department. You can print out the copy of the application form available on the web site and fill it out ahead of time.
Show a current state issued picture I.D.
The fee is $30 for the first document and $10 for each additional copy for that individual.
It can be processed while you wait.
Records are available back to 1977. Older records must be requested from the Office of Vital Records and Statistics, 288 N 1460 W, P.O. Box 141012, Salt Lake City, UT 84114-1012. The time frame is 3 to 4 weeks. These certificates can be expedited in a 24-hour time period for a fee of $45.
