How to Apply:

1. Fill out application at your local health department. You can print out the copy of the application form available on this web site and fill it out ahead of time.
2. Show a current state issued picture ID.
3. The fee is $16 for the first document and $8 for each additional copy for that individual.
4. It can be processed while you wait.
5. Records are available back to 1977. Older records must be requested from the Office of Vital Records and Statistics, 288 N 1460 West, P.O. Box 141012, Salt Lake City, UT 84114-1012. The time frame is 3 to 4 weeks. These certificates can be expedited in a 24 hour time period for a fee of $28.
6. For genealogy purposes, contact your local office for further information.
